Pre-Registration is currently only available online for patients preparing to have a baby.
You can save time before you go into labor by Pre-Registering, at 30 weeks, before your expected due date for delivery.
There are several ways to pre-register:
In person
You may pre-register on the 2nd Floor of the Women's Center (Entrance #2) at the Labor and Delivery front desk between the hours of 7:30 am - 5:30 pm, Monday through Friday. A photo I.D. and insurance card will be required.
By mail
You may pre-register by completing the printable form and mailing it to the following address with a copy of your photo I.D. and insurance card.
